Bassetlaw Intermediate Care Services

What is Bassetlaw Intermediate Care?

Bassetlaw Intermediate Care is a joint service provided by Nottinghamshire Social Services, Doncaster & Bassetlaw Hospitals NHS Trust and Bassetlaw Primary Care Trust. It is for people who, with the benefit of further rehabilitation and recuperation in a residential setting, may then be able to go home.

Westwood is a 60 bed residential home. 15 of the beds are dedicated to Bassetlaw Intermediate Care. You will have your own bedroom with en-suite facilities, but share kitchen, lounge and dining room facilities with some other residents.

What does the scheme involve?

Short term rehabilitation support will be provided by dedicated staff and will be based around activities of daily living. With appropriate support you will be encouraged to be as independent as possible with your personal care, kitchen activities and mobility. The therapy staff will discuss a plan of care with you and will relate it specifically to your home environment and daily routine.

How long will I be on the scheme?

You could stay for a maximum of 6 weeks and there will be regular meetings with you and the rehabilitation team to discuss how you are getting on. If you are ready to go home after a shorter length of time than was originally planned, this will be arranged.

What happens at the end of the scheme?

During the final week of your stay arrangements will be made for your return home with equipment/support services as required. However, if it becomes evident that you will not be ready to return home in the fourth week, alternative arrangements will have to be made.

What will the cost me?

There will be no charge for personal care whilst you are receiving the Intermediate Care Service. There will be a charge for services such as hairdressing, personal telephone etc. If you remain in Westwood when your period of Intermediate Care has finished, you will be asked to co-operate in the completion of a financial assessment in order to determine how much you will have to contribute towards your on-going period of care.

What if I change my mind?

If at any time prior to transfer to Bassetlaw Intermediate Care you have any doubts and wish to change your mind you may do so.

What are the benefits of the scheme?

  • Your family and carers will be given an opportunity to be more closely involved in your rehabilitation programme.
